For New and Established Patients
In order to better serve our patients all medical appointments are made online.
Phone calls to schedule or re-schedule appointments will no longer be handled over the phone for existing patients.
Established Patient – Follow-Up Consultation
If you are an established patient of Dr. Blackman and you have had an appointment within the past 2 years, you may proceed with scheduling a “Medical Office Consult – Established Patient” or “Medical Phone Consult – Established Patient” through our online scheduling system below. If you were instructed to get lab work done prior to your appointment, please make sure there are at least 8 days between the date of your blood draw and the date of your appointment so the results can be reviewed by the doctor prior to your appointment. Do not call our office to confirm that your lab results are complete. If we have not received the lab results within 48 hours from your scheduled appointment date we will contact you directly.
New Patient – Initial Consultation
If you will be a new patient of Dr. Blackman or it has been more than 2 years since your last appointment, the FIRST STEP is use the on-line scheduling system below and select "Medical Office Consult - Initial" or "Medical Phone Consult - Initial". You will receive an email confirming your appointment REQUEST. The SECOND STEP is to confirm your appointment request with a required non-refundable $100 deposit.
Once your payment is received we will approve your appointment request and you will receive a confirmation email to that effect. THIRD STEP: New patients, as well as patients who have not been seen for over 2 years are required to complete a New Patient Intake Packet and send it in at least two business days prior to your appointment.
The completed packet may be faxed (240-235-3571), emailed to email@example.com, or sent to us via any other delivery service or U.S. mail. Please note we are closed on Sundays and Mondays.
Please do not schedule an Initial Medical Consult within less than 4 business days from the time of your request. It is important to allow sufficient time for your appointment to be confirmed and for you to complete the New Patient Intake Packet.
